I had to drop by the Davis County Health Department Office of Vital Records to get a certified birth certificate. The staff was very helpful, even though I didn't have all the information I needed. First-rate service.

I've had to come here a few times to pick up birth certificates. Although I generally dread having to walk into a government office as a customer (even though I'm actually a government employee myself), each visit was quick and productive. It's also a nice, clean facility.
do not take your kids to this please to get there shots done they are not sanitary it not a clean environment
